Diabetic Rehabilitation Program
Because diabetes must be managed daily, knowing how to control your blood sugar is critical to your health. Diabetes Educators with Brooksville Regional Hospital & the
Hernando County Health Department/Nature Coast Community Health Center can give you the practical tools and knowledge you need to fit diabetes into your life.
Nationally accredited by the
American Diabetes Association, the Diabetes Self Management Program is designed to promote goal-directed, comprehensive education for adults with the diagnosis of diabetes. The education program is coordinated by a Certified Diabetes Educator and the exercise component by a Registered Nurse.
Sequenced Diabetes Education
Patient education is an integral part of the Diabetic Rehab/ Self Management Program. It provides you with an individualized pathway of care based on your particular needs. It is about empowering you to self-manage the day-to-day care of your diabetes. Our program offers group classes to help patients understand their disease. Patients benefit from time with both a dietitian and nurse as well as group classes and follow-up sessions when needed.
Class content, presented in weekly sessions includes:
· Overview of disease process
· Nutrition, planning diet Exercise
· Managing high and low blood sugars Medications
· Preventing complications of diabetes Goal setting
· Foot & skin care Blood glucose monitoring & target ranges
The program is designed for people who:
- Have been recently diagnosed Have diabetes but have never participated in an education program
- Have trouble controlling their blood sugar levels or
- May be suffering from diabetes-related complications difficulty
Individually Designed Exercise Program
It is important for diabetic management and overall lifestyle improvement that you learn how to incorporate safe exercise into your activities of daily living. Exercise classes will last about one hour, twice a week and consist of low-level, low intensity exercise tailored to your individual needs and limitations. Your exercise plan will be based on an individual assessment by one of our program specialists. Throughout the program, your exercise component will be adjusted as progress is made.
How to enroll
Talk with your primary care physician he or she will need to: Send a referral request to Brooksville Regional Hospital Diabetic Rehabilitation Department. Most health insurance covers this program. As a participant, you are responsible for a $50 registration fee, which is not covered under insurance.

Diabetic Support Group
The Diabetic Support group meets the 2nd Monday of each month at 4p.m. in the 1st floor conference rooms. This is open to the public - you do NOT have to be in our Diabetic Rehabilitation program to come. For more information, please call 352-796-5111, ext. 4317.
